Mutual Client Update.
At UIMH, the care and treatment progress of our client’s are our top priority. This form seeks updates on the specific services rendered to the mutual client, current progress, any challenges faced, and other pertinent details that can help in comprehensively assessing the client's journey and well-being. To remain a continued referral source for UIMH, we require that a monthly mutual client update be submitted for each client of ours you serve.
