FAQs

Why do I have to fill out paperwork?

At UIMH, we believe that in order to best serve you we must get as much information before you enroll and during your diagnostic evaluation as possible. Rendering the correct diagnosis is essential to being able to provide you with effective treatment. A misdiagnosis could significantly hinder your ability to heal and thrive. There is also legally required and insurance mandated paperwork that you must complete.

How long will it take for me to complete my paperwork?

You can expect to spend between 15-30 minutes completing your paperwork.

What happens if I do not complete my paperwork prior to my first session?

You must have your paperwork completed at least 24-hours prior to the appointment so that your therapist can come prepared with the right assessment questions for you. We will give you multiple reminders to complete your paperwork via phone, text, and email. If your paperwork is not completed, your appointment will be cancelled and you will not be permitted to reschedule until it has been completed.

How often should I be seen as a new client?

It is typically best for you to attend session once per week for the first 6 months during your initial treatment phase. We have found that creating a recurring appointment with your clinician is best practice. If you prefer to schedule on your own, you may do so using your patient portal.
(Please note that you must be seen at least once per month).
